All About Me Project

We can assist in completing the first step of the Education, Health and Care Plan: the 'All About Me' section.

The 'All About Me' section is vital as it gives the individual the opportunity to express their views, interests and aspirations, and lay the foundations for a care plan that is centred around their personality.

Questions in the 'All About Me' section include:

​

• What is important to me?

• What am I good at?

• What do I find difficult?

• What makes me happy?

• What would I like to achieve in the future?

​

It will also determine and display the best way to communicate with the individual and highlight any behaviours that might be misunderstood or misinterpreted.
